Changing the Trach

A.  Reasons to change

1. Routine change monthly

- The trach tube should be changed monthly

- The trach can be changed by a (trained) parent/guardian, home health nurse or provider in clinic

- Change the trach tie at least once a day and anytime it is soiled or wet

2. Emergency:

- Accidental decannulation (dislodgement of trach tube)

- Obstructed trach tube (mucus plug)

i. Patient will have impaired ventilation and potentially respiratory distress

ii. Suction catheter is not able to pass/clear the osbtruction

iii. Unable to adequately ventilate with ambu bag (6)
